Job summary

Fuze Health is seeking a Registered Nurse to support the telehealth care team by providing remote outreach to Medicare members and ensuring compliance with a new Stars measure. The role includes ED discharge follow-ups and other HEDIS tasks, delivered with compassion and high-quality virtual care.

Schedule: Monday–Friday 12:30pm–9pm EST plus one Saturday per month. Requires AZ RN license and 2+ years of experience, with strong EMR skills and customer service focus.

Qualifications

• BSN degree required.
• Active, non-restricted nursing license; primary AZ or compact state license.
• 2+ years of RN experience.
• 1–2 years remote/telehealth experience.
• Strong computer skills and EMR familiarity.
• Clear verbal and written communication.
• Able to work independently and in a team.
• Startup experience preferred.

Responsibilities

• Provide telehealth patient care via remote platforms.
• Conduct virtual patient assessments with histories and symptoms.
• Document patient conditions and care interactions.
• Educate patients and families on conditions, meds, and self-care.
• Collaborate with providers to develop care plans.
• Maintain regulatory compliance and privacy adherence.
• Stay updated on telehealth protocols and best practices.
• Demonstrate compassionate and professional communication.
