Job summary

Sprenger Therapy seeks an Ohio-licensed Physical Therapist to provide telehealth services to residents within our skilled nursing communities. You’ll deliver patient-centered care through secure video technology while collaborating closely with on-site therapy teams.

Qualifications

• Must hold an Ohio Physical Therapist license or be eligible for licensure.
• Degree in Physical Therapy (Bachelor's, Master's or Doctorate) from an accredited program.
• Experience in skilled nursing, geriatrics, post-acute care, or telehealth preferred.

Responsibilities

• Perform physical therapy evaluations and reassessments via secure telehealth.
• Develop individualized care plans with residents and interdisciplinary team.
• Provide skilled telehealth treatment focusing on mobility, strength, balance, safety, and independence.
• Collaborate with on-site staff during evaluations and treatments.
• Supervise Physical Therapist Assistants as permitted by Ohio law and payer requirements.
• Participate in care planning and discharge planning.
• Identify residents who may benefit from PT intervention and communicate with the care team.
• Document services clearly and efficiently, ensuring resident-centered care.
