Sprenger Therapy is seeking an Ohio-licensed Physical Therapist to provide telehealth services to residents within our skilled nursing communities. You will deliver patient-centered care via secure video and collaborate closely with on-site teams, maintaining the clinical integrity and outcomes-driven focus of our family-owned organization.

Qualifikationen

• Must hold a current Ohio Physical Therapist license or be eligible for licensure.
• Experience in skilled nursing, geriatrics, post-acute care, or telehealth preferred.
• Excellent communication skills and ability to build rapport via video.
• Ability to work independently with strong clinical judgment.

Aufgaben

• Perform telehealth PT evaluations and reassessments via secure video.
• Develop individualized care plans with residents and interdisciplinary team.
• Provide skilled telehealth treatment focusing on mobility, strength, balance, safety, function, and independence.
• Collaborate with on-site staff during evaluations and treatments.
• Supervise PTAs as permitted by Ohio law and payer requirements.
• Contribute to care planning and discharge planning.
• Document services clearly and efficiently in the EHR.
• Maintain resident-centered, engaging telehealth experiences.
