Job summary

Sprenger Therapy is seeking an Ohio-licensed Physical Therapist to provide telehealth services to residents within our skilled nursing communities. You’ll deliver meaningful, patient-centered care through secure video technology while collaborating closely with our on-site therapy teams.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Current Ohio Physical Therapist license or eligibility for Ohio licensure.
• Experience in skilled nursing, geriatrics, post-acute care, or telehealth preferred.
• Strong clinical judgment and ability to work independently.
• Clear communication skills and ability to build rapport through video.
• Team-oriented mindset and ability to collaborate with on-site staff.
• Comfort using electronic documentation and video technology.
• Reliable high-speed internet and private workspace for confidential care.
• Commitment to patient-first, outcomes-driven care.

Responsibilities

• Complete physical therapy evaluations and reassessments through secure telehealth technology.
• Develop individualized plans of care in collaboration with residents, families, and the care team.
• Provide skilled telehealth treatment focused on mobility, strength, balance, safety, function, and independence.
• Collaborate with on-site therapy staff during evaluations, treatments, measurements, and assessments.
• Supervise and provide clinical direction to Physical Therapist Assistants, as permitted by Ohio law.
• Participate in care planning and discharge planning.
• Conduct screenings and identify residents who may benefit from physical therapy.
• Communicate with residents, families, nursing staff, physicians, and care team.
• Document services clearly, accurately, and efficiently.
• Create a telehealth experience that remains personal, engaging, and resident-centered.
