Job summary

Sprenger Therapy is seeking an Ohio-licensed Physical Therapist to provide telehealth services to residents within our skilled nursing communities. This remote role connects you with an on-site interdisciplinary team, ensuring mobility and function goals are met via secure video visits.

You’ll evaluate, plan, and implement care with strong clinical autonomy while collaborating closely with residents, families, and staff for meaningful outcomes.

Qualifications

• Ohio license as Physical Therapist or eligibility for licensure.
• Experience in skilled nursing, geriatrics, post-acute care or telehealth preferred.
• Strong clinical judgment and ability to work independently.
• Clear communication skills and ability to build rapport through video.
• Reliable high-speed internet and private workspace for confidential care.

Responsibilities

• Complete physical therapy evaluations and reassessments via secure telehealth.
• Develop individualized plans of care with residents, families and the interdisciplinary team.
• Provide skilled telehealth treatment focused on mobility, strength, balance, safety, function and independence.
• Collaborate with on-site therapy staff during evaluations, treatments, measurements and assessments.
• Supervise PT Assistants, as permitted by Ohio law and payer requirements.
• Document services clearly, accurately and efficiently.
• Create a telehealth experience that remains personal, engaging and resident-centered.
