Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide telephonic or video-based counseling and MTM services from a home office. You will engage patients and caregivers, monitor therapy, and support adherence in a flexible, work-from-home schedule.

Ideal candidates hold a PharmD or Bachelor of Pharmacy, an active US pharmacist license, and experience in ambulatory care or MTM. Multiple state licenses are a plus but not required.

Formación

• Pharmacist license active in the U.S.; multiple state licenses preferred but not required.
• Ambulatory care, MTM, or clinical experience preferred; hospital/community pharmacists with patient interaction interest encouraged.

Responsabilidades

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document all interventions and communications according to organizational standards.
• Support population-health and disease-management programs.
