Qualifications
- PharmD or Bachelor of Pharmacy is required.

- Ambulatory care, MTM, or clinical pharmacy experience is preferred.
- Excellent communication and ability to engage patients virtually.
Responsibilities
- Deliver telephonic or video-based medication counseling and education to patients and caregivers.
- Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
- Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
- Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
- Document all interventions and communications according to organizational standards.
- Support population-health and disease-management programs.
