Pear Suite is seeking a Nurse Practitioner for a virtual, PRN role that scales with patient demand. You will conduct telehealth evaluations, determine service referrals, and develop care plans to support CHW services and community-based care.

The role starts on a PRN basis with a pathway to more consistent, long-term work as the volume and footprint grow, offering a flexible schedule and a fully remote setup.

Qualifications

• Active, unrestricted Nurse Practitioner/APRN license and prescribing authority in practice state(s).
• Licensure in at least one of: Alaska, Arizona, California, Louisiana, New Mexico, Nevada.
• Experience in primary care, family medicine, internal medicine, urgent care, or chronic disease management; telehealth a plus.
• Experience with Medicaid/Medicare populations.
• Spanish or other language fluency is strongly preferred.

Responsabilités

• Conduct telehealth clinical evaluations to assess health needs and social drivers of health.
• Determine clinical appropriateness for CHW services and refer for care coordination.
• Develop individualized care plans addressing clinical and social determinants of health.
• Collaborate with Community Health Workers to support patient engagement.
• Address acute, chronic, preventive, and medication-management needs within program scope.
