Job summary

Pear Suite is a remote, mission‑driven digital health company delivering care navigation tools to frontline workers across the U.S. This Nurse Practitioner role starts PRN with a flexible schedule and a clear pathway to regular part‑time or full‑time work as volumes grow.

You will assess health needs via telehealth, enable CHW services, and coordinate referrals to social and medical resources, aiming to improve health outcomes and patient experience.

Qualifications

• Active, unrestricted Nurse Practitioner/APRN license in the state(s) where care is provided.
• Experience in primary care, family medicine, internal medicine, urgent care, or chronic disease management.
• Telehealth experience a plus.
• Spanish or other language fluency strongly preferred.
• Experience with Medicaid/Medicare populations.

Responsibilities

• Conduct telehealth clinical evaluations to assess health needs and social drivers of health.
• Determine clinical appropriateness for CHW services and make referrals for care coordination.
• Develop individualized care plans addressing clinical needs and social determinants of health factors.
• Collaborate closely with Community Health Workers to support patient engagement and follow‑through.
• Address acute, chronic, preventive, and medication-management needs within program scope, including non‑controlled meds when indicated.
• Document visits accurately and timely per requirements.
• Help patients navigate complex healthcare systems and connect to resources.
