Job summary

Greater Good Health is seeking a Nurse Practitioner for telehealth to deliver high-quality care to older adults in a value-based model. This contractor role is full-time and remote, with a Monday–Friday 8am–5pm schedule and start ASAP; Alabama NP licensure is required.

The role focuses on annual wellness visits, high-risk patient management, and coordination with a Primary Care team. We offer license reimbursements, malpractice coverage, and a supportive NP community to advance professional

Qualifications

• NP certification from ANCC/AANP
• 2+ years of NP experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care (home care a plus)
• Working knowledge of chronic care management and various medication options
• Current DEA licensure / Prescriptive Authority post-hire
• Active CPR/BLS Certification
• Knowledge/history working with HCCs

Responsibilities

• Conduct annual wellness visits and manage high-risk patients primarily via telehealth (possible in-home visits)
• Perform transition of care visits to ensure discharge instructions are understood and barriers to recovery are removed
• Document medical histories, symptoms, and medication information
• Administer tests and screenings
• Identify care gaps and communicate interventions to the member’s primary care provider and care team
• Collaborate with the Primary Care team for holistic patient care
• Ensure timely completion of patient records with access to EMR systems
• Provide patient education on preventive care and prescribed treatments
• Maintain HIPAA confidentiality
