Job summary

Greater Good Health is hiring a Part-Time Nurse Practitioner in Dothan, AL to deliver high-quality, patient-centered care. You will split time between home visits and telehealth, conducting wellness visits, managing chronic conditions, and coordinating with the Primary Care team.

Qualifications

• NP certification from ANCC or AANP.
• Minimum 2 years clinical NP experience in primary care, family medicine, internal medicine, gerontology or palliative care.
• Active DEA license post-hire.
• Active CPR/BLS certification.
• Knowledge of HCCs.

Responsibilities

• Conduct annual wellness visits and manage chronic conditions via a mix of in-home and telehealth visits.
• Perform transition of care visits to ensure understanding of discharge instructions and reduce avoidable readmissions.
• Thoroughly document medical histories, symptoms, and medication information.
• Administer various tests and screenings.
• Identify care gaps and communicate recommendations to the member’s PCP and care team.
• Collaborate with the Primary Care team to ensure holistic patient care.
• Ensure timely completion of patient records with access to EMR systems.
• Provide patient education on preventive care and prescribed treatments.
• Maintain confidentiality in accordance with HIPAA regulations.

The Role:
As a Nurse Practitioner at Greater Good Health, you'll be at the heart of our mission to provide exceptional care to our patients. We want our clinicians to take the time needed to address the patient's needs and concerns. Your responsibilities will include:

• Conducting annual wellness visits/managing chronic conditions via mix of in-home and telehealth visits
• Perform transition of care visits to ensure patient understanding of discharge instructions, remove barriers for recovery and reduce avoidable hospital readmissions or emergency department visits.
• Thoroughly documenting medical histories, symptoms, and medication information
• Administering various tests and screenings
• Identify care gaps and communicate recommended interventions to the member’s primary care provider and care team.
• Collaborating with the Primary Care team to ensure holistic patient care
• Ensuring timely completion of patient records, with access to electronic medical records and portals
• Providing patient education on preventive care and prescribed treatments
• Maintaining strict confidentiality in line with HIPAA regulations

What You Can Expect:
This is a part-time Nurse Practitioner position (2-3 days/week, 8am–5pm) combining telehealth visits with in-home visits. Home visits are scheduled within approximately one-hour travel radius of Dothan, AL.

Experience and Qualifications

• Nurse Practitioner certification from either the ANCC / AANP
• 2 + years of clinical experience as a NP in either Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care. Home care experience is a plus
• Working knowledge of chronic care management and various medication options
• Current DEA licensure / Prescriptive Authority post-hire
• Active CPR/BLS Certification
• Must have knowledge/history working with HCCs
