Job summary

Greater Good Health is seeking a Part-time Nurse Practitioner to join our team in Birmingham, AL. The role combines in-home and telehealth visits, with 2–3 days per week (M-F 8-5). You will conduct wellness visits, manage chronic conditions, and collaborate with primary care teams to improve outcomes.

Applicants should have ANCC/AANP NP certification, 2+ years of NP experience, DEA licensure, CPR/BLS, and a commitment to HIPAA privacy. We offer a supportive community and competitive compensation.

Qualifications

• Nurse Practitioner certification from ANCC or AANP.
• 2+ years of clinical NP experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care; home care experience is a plus.

Responsibilities

• Conduct annual wellness visits and manage chronic conditions via in-home and telehealth visits.
• Document medical histories, symptoms and medications accurately.
• Perform tests and screenings and ensure timely documentation in EMR.
• Identify care gaps and communicate recommended interventions to the primary care team.
