Job summary

Greater Good Health invites a Part-Time Nurse Practitioner to join our team in Auburn, AL. The role blends telehealth visits with in-home visits, with 2–3 days per week, typically 8am–5pm. Our clinicians address patient needs with time for comprehensive care and clear documentation in EMRs.

You will conduct wellness visits, manage chronic conditions, and coordinate with the Primary Care team to close care gaps while maintaining HIPAA standards. Travel within ~1 hour of Auburn is typical.

Qualifications

• NP certification from ANCC or AANP.
• 2+ years of NP clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care.
• Current DEA licensure/prescriptive authority post-hire.
• Active CPR/BLS certification.
• Knowledge/history working with HCCs.

Responsibilities

• Conduct annual wellness visits and manage chronic conditions via in-home and telehealth visits.
• Perform transition of care visits to ensure understanding of discharge instructions and reduce readmissions.
• Document medical histories, symptoms, and medication information.
• Administer various tests and screenings.
• Communicate care gaps and recommended interventions to the PCP and care team.
• Collaborate with Primary Care to ensure holistic patient care.
• Ensure timely completion of patient records with EMR access.
• Provide patient education on preventive care and prescribed treatments.
