Job summary

Catapult Health is hiring a PRN Nurse Practitioner to conduct private preventive assessments via telehealth, while working from our Dallas, TX office. You will educate patients on risk factors for diabetes, heart attack, and stroke, document in Catapult’s EMR, and tailor an individualized action plan based on results.

Requirements include TX RN and APRN licenses, national NP certification, ongoing multi-state licensing, 1+ year of primary care/internal medicine NP experience, and strong

Qualifications

• TX RN license (Compact) is required.
• TX state APRN license is required.
• National NP certification (FNP, AGNP or ANP) is required.
• Must be licensed in at least 1 other state as NP.
• Willing to obtain 3–5 additional state licenses quickly.
• Minimum 1 year NP (Primary Care/Internal Medicine) experience.
• Strong computer and communication skills.
• Spanish bilingual is a strong plus.

Responsibilities

• Conduct private preventive assessments via Catapult video conference.
• Educate patients on diabetes, heart disease, and stroke risk.
• Document consultations in Catapult EMR.
• Create individualized action plans based on results.
• Assess depression risk and refer to resources.
• Refer patients to employer resources as needed.
• Engage patients to commit to a forward health plan.
• Counsel using current clinical guidelines (AHA, ACC, etc.).
