Job summary

Devoted Health is hiring a Preventative Primary Care Provider (NP/PA) in the United States, with telehealth visits and limited in-home follow-ups. You’ll deliver comprehensive preventive care, chronic condition support, and medication optimization within a tech-enabled team.

A strong emphasis is placed on member-centric, whole-person care and collaboration across care teams. Active NP/PA licensure with BLS, 3+ years outpatient experience, and comfort with video visits are required.

Qualifications

• Active APRN/NP or PA license.
• 3+ years outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics.
• Active and clear NP/PA license (willing to obtain additional licenses within 90 days).
• Active BLS certification at time of hire.
• Comfort with delivering care via video telehealth and using electronic documentation systems.

Responsibilities

• Conduct primarily telehealth video visits, with limited in-home visits.
• Perform comprehensive assessments and provide evidence-based care focused on prevention and chronic condition support.
• Identify and address care gaps, coordinate with care team, and contribute to holistic care plans.
• Utilize technology and AI-enabled workflows to reduce admin burden and maximize patient interaction time.
• Communicate with empathy and clarity to foster trust with members and caregivers.
