Devoted Health is hiring a Telehealth Preventative Primary Care Advanced Practice Provider (NP/PA) to join our mission-driven clinical team. You will conduct telehealth visits, perform comprehensive assessments, support chronic condition management, and emphasize prevention and proactive care for our members in a tech-enabled environment.

We seek an APRN/NP or PA with 3+ years of outpatient practice, BLS certification, and comfort with telehealth. Some in-home visits may occur in the local area.

Qualificações

• Active APRN/NP or PA license.
• 3+ years of outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics.
• Active BLS certification at time of hire.
• Comfort with delivering care via video telehealth and using electronic documentation systems.
• Willing to obtain additional licenses if necessary within first 90 days; support and reimbursement provided.

Responsabilidades

• Conduct telehealth video visits with patients.
• Perform comprehensive clinical assessments and provide preventive and chronic care management.
• Identify and address care gaps; coordinate with care team and families.
• Utilize AI-enabled workflows to reduce administrative burden and maximize patient interaction time.
