Urban Health Plan, Inc. is seeking a remote Medical Assistant to manage messages, phone encounters, and assist the nurse in Bronx-area care coordination. The role includes monthly onsite requirements in NYC and commuting to the NYC job market as needed.

Qualifications

• High School Diploma required.
• Medical Assistant certificate or accredited program completed.

Responsibilities

• Prepares patients for care, conducts screenings, and facilitates discharge.
• Verifies patient demographics, pharmacy, and insurance.
• Assists with scheduling for call center patients.
• Assist with post-discharge appointments, referrals, DME orders.
• Collects discharge records, medical histories, and treatment plans.
• Addresses provider telephone encounters and messages.
• Collaborates with nurse for clinical guidance as needed.
• Performs other job-related duties.
