Job summary

Urban Health Plan seeks a Remote Medical Assistant for the Virtual Care Center in NYC area. You will manage messages, telephone encounters, and assist the nurse to ensure timely patient care.

The role is remote, with monthly in-person activities in the Bronx and as-needed onsite requirements in New York City. Requirements include a High School Diploma, MA certificate or accredited program, 2 years MA experience, bilingual Spanish/English, strong computer skills, and excellent communication.

• Collect discharge records, medical histories, and treatment plans.
• Address provider telephone encounters and messages.
• Collaborates with nurse and seeks clinical guidance as needed.
• Perform other related duties as required.

Position Summary: The Virtual Care Center (VCC) provides rapid access to telehealth appointments for patients with acute medical conditions requiring same-day care. The VCC Medical Assistant is a remote role responsible for managing messages, handling telephone encounters, and assisting the nurse as needed to ensure efficient patient care and communication. Although this is a remote role- incumbents are required to come in monthly to work activities in the Bronx, NY and requires as-needed onsite requirements in New York City. Accordingly, applicants must be in a commutable vicinity to the NYC job market. Minimum Qualifications: High School Diploma required. Previous experience working in a community-based health center or similar setting. Medical Assistant Certificate or completion of an accredited Medical Assistant program required. Bilingual Spanish/English required. 2 years of established Medical Assistant experience. Flexible work schedule. Proficiency with computers and software. Strong communication skills. Ability to multi-task.

Key Duties:

• Prepares patients for care, conducts screenings, and facilitates discharge process.
• Verifies patient demographics, pharmacy, and insurance.
• Assists in ensuring that call center patients are scheduled appropriately on the template.
• Assist with post discharge appointments, referrals, DME orders.
• Assist with the collection of discharge records, medical histories, and treatment plans.
• Address provider telephone encounters, messages plus, etc.
• Collaborates with nurse and reaches out for clinical guidance when appropriate.
• Perform other job-related duties as required

Compensation: $23.00/hour for 40 regular hours per week. Work Environment Type, Location, and Schedule: Candidates must possess availability to work remotely. Required schedule: Mondays 1pm-10pm, Tuesday 10am-10pm, Wednesdays 2pm-10pm, Thursdays 3pm-10pm, Fridays 1pm-10pm.
