Job summary

SynergenX Health & Wellness Clinic is seeking a full-time Telemedicine Provider (NP or PA) to care for patients in Indiana through telehealth. The role requires an active Indiana FNP or PA license, Indiana CSR, and Full DEA registration, with a fully remote, W-2 employment arrangement.

The provider will deliver evidence-based hormone optimization and preventive care, serving both men and women, and will have opportunities for professional growth as the practice expands.

Qualifications

• Strong clinical assessment and medical decision-making skills.
• Exceptional patient communication and relationship-building abilities.
• Active Indiana FNP license with Indiana APRN licensure (Family) or Indiana PA license under your NPI taxonomy.
• Current Indiana CSR and FULL DEA registration (at time of application; no reimbursement).
• Current National Provider Identifier (NPI) number.
• Current board certification applicable to your profession.
• NPI number and board certification information must be disclosed during the application process.
• No current or pending malpractice lawsuits.
• Experience or strong interest in hormone replacement therapy, women’s health, or preventive medicine preferred.
• Bilingual English/Spanish highly preferred.

Responsibilities
