SynergenX Health & Wellness Clinic is seeking a full-time Telemedicine Provider (NP or PA) to care for patients in Indiana through telehealth. The role requires an active Indiana FNP or PA license, Indiana CSR, and Full DEA registration, with a fully remote, W-2 employment arrangement.

The provider will deliver evidence-based hormone optimization and preventive care, serving both men and women, and will have opportunities for professional growth as the practice expands.

Qualifications

• Strong clinical assessment and medical decision-making skills.
• Exceptional patient communication and relationship-building abilities.
• Active Indiana FNP license with Indiana APRN licensure (Family) or Indiana PA license under your NPI taxonomy.
• Current Indiana CSR and FULL DEA registration (at time of application; no reimbursement).
• Current National Provider Identifier (NPI) number.
• Current board certification applicable to your profession.
• NPI number and board certification information must be disclosed during the application process.
• No current or pending malpractice lawsuits.
• Experience or strong interest in hormone replacement therapy, women’s health, or preventive medicine preferred.
• Bilingual English/Spanish highly preferred.

Full-Time Telemedicine Provider (NP or PA) - Servicing Indiana (fully Remote)

SynergenX is seeking a full-time Telemedicine Provider to care for patients throughout Texas via telehealth. Candidates must hold an active Indiana license and a current Indiana CSR and Full DEA registration. This is a fully remote, full-time W-2 position.

SynergenX is a leader in evidence-based hormone optimization and wellness care, providing comprehensive treatment for both men and women. While this position will treat patients of both biological sexes, including comprehensive hormone management and medical weight loss.

This is an exciting opportunity for a compassionate, patient-focused Physician Assistant (PA) or Family Nurse Practitioner (FNP) who is passionate about helping patients improve their quality of life through personalized hormone optimization and preventive healthcare. As SynergenX continues to grow, there are significant opportunities for professional development and career advancement.

We welcome applications from both experienced providers and newer graduates who have already obtained their Texas DEA registration and are ready to begin practicing independently. Candidates must possess an active DEA at the time of application, as the company does not reimburse or sponsor DEA registration costs.

We are seeking a provider who is available to begin employment with minimal delay or no need of transfer. Candidates who can complete the hiring and credentialing processpromptly (NPI active, board certification active with proof, License fully unencumbered, CSR/DEA active for IN) are given first priority.

Requirements

• Active IndianaFamily Nurse Practitioner (FNP) license with Indiana APRN licensure (Family specialty only) or an active Indiana Physician Assistant license under your primary taxonomy NPI.
• Current Indiana CSR and FULLDEA registration (required at the time of application; no company reimbursement is provided).
• Current National Provider Identifier (NPI) number.
• Current board certification applicable to your profession.
• NPI number and board certification information must be disclosed during the application process to be considered for this opportunity.
• No current or pending malpractice lawsuits.

Schedule

• Full-time, Monday through Friday.
• Patients are scheduled between 8:00 AM and 5:00 PM Eastern Time.
• Fully remote while providing telemedicine services to patients located throughout Indiana.

Training

• Seven days of paid remote training, everything you need in order to effectively and confidently begin seeing patients.

Compensation

• $120,000 annually.
• Full-time, exempt W-2 position.
