Job summary

The SWAN Adoption Coordinator at Gladney Center For Adoption Inc will coordinate the statewide adoption network and post-permanency services, providing case management before and after placement and supporting the adoption program unit.

This role requires engaging with families and partners, maintaining records, producing reports, and representing the agency at events while traveling locally.

Qualifications

• Bachelor’s degree in Social Work or closely related field required; MSW preferred.
• State social work licensure preferred and may be required depending on caseload.
• Minimum of six months of direct SWAN adoption and family services experience.
• Proven experience delivering public presentations and representing an organization professionally.
• Excellent written communication skills with ability to prepare clear reports and documentation.
• Strong verbal communication with families, stakeholders, and partner agencies.
• Demonstrated problem-solving abilities with sound judgment and decision-making skills.
• Skilled in facilitating meetings and services involving families and children.
• Comfortable with public speaking to diverse audiences.
• Strong organizational, planning, and time management skills to manage multiple cases.

Responsibilities

• Coordinate the SWAN and Post Permanency Services programs to meet all requirements.
• Coordinate completion of child preparation services as referred.
• Update and maintain accurate records for assigned clients.
• Complete SWAN benchmark reporting and related documentation.
• Ensure documentation and benchmark reports comply with contracts.
• Coordinate completion of child and family profiles as referred.
• Coordinate finalization services as referred.
• Manage an assigned SWAN and Post Permanency Services caseload.
• Assist in building and expanding the SWAN program and representation of Gladney.
• Attend adoption-related events to earn continuing education credits.
• Represent the agency at SWAN meetings and public affairs events.
• Provide statistics and reports on SWAN activities as needed.
• Provide backup support to other social work positions as needed.
