Job summary
Holon Health is seeking an experienced Nurse Practitioner licensed in Arizona to support individuals with SUD through collaborative, integrated care. You will work with Care Navigators, Recovery Coaches, Case Managers, and community partners to address the full range of mental, emotional, and physical needs.
The role offers remote flexibility and can be full-time (40 hours/week) or part-time (20 hours/week).
Qualifications
- Licensed NP with Arizona credential and unrestricted practice.
- DEA license active; CDS where applicable.
- Experience in behavioral health, addiction medicine, or psychiatry.
- Board certification in specialty is preferred.
- Strong documentation skills and knowledge of EHR systems.
Responsibilities
- Conduct telehealth evaluations and care coordination.
- Prescribe and manage medications; monitor effects.
- Collaborate with care team and external partners for integrated care.
- Serve as clinical consultant on complex cases.
- Educate clients and families on treatment plans and recovery supports.
- Maintain compliant documentation and participate in ongoing training.
- Engage in team meetings and professional development.
