Qualifications

• Active and verifiable license from an Arizona license (LCSW, LPC, LMFT, LIAC, LACT, or LAAC) and state of residence if not Arizona; credentials verified via State Board website.

Responsabilités

• Conduct intake assessments to evaluate Member readiness and determine appropriate level of care.
• Facilitate group and one-on-one counseling sessions using Affect’s evidence-based curriculum.
• Provide crisis support and coordinate appropriate referrals when needed.
• Maintain timely, accurate documentation in the EMR system.
• Collaborate with clinical supervisors on individualized treatment and discharge planning.
• Ensure compliance with HIPAA and all relevant state regulations.
