Job summary

CommonSpirit Health is hiring an APP Spine Program Navigator to coordinate care for patients referred to the Back Pain/Spine Program. You will lead a multidisciplinary team, promote resource use, and remove barriers to ensure high-quality spine care for patients and families.

You will travel within the Denver metro area to cover affiliated hospitals, work primarily remote, and support education for staff and patients while maintaining evidence-based care protocols.

Qualifications

• Master’s Degree in Nursing as NP from a nationally accredited postgraduate program.
• Minimum of two years caring for surgical spine patients.
• Minimum of two years of clinic experience.
• Registered Nurse with APRN board certification and DEA registration.

Responsibilities

• Develop and lead performance improvement initiatives related to quality and patient, physician, and staff satisfaction.
• Demonstrate leadership and commitment to excellence in patient care and service.
• In collaboration with the interdisciplinary team, develop, maintain, and monitor spine practice guidelines, order sets, and clinical pathways based on evidence-based medicine.
• Develop and nurture collaborative working relationships that create a sense of ownership in the spine service line with hospital department leaders, orthopedic spine surgeons, neurosurgeons, and other staff and specialists involved in the care continuum for spine patients.
• Monitor patient care outcomes for spine services.
• Promote and support physicians and other staff participation in collaborative practice.
