Job summary

Optum Home & Community Care, part of the Optum family, is hiring a Medical Social Worker to assess psychosocial needs and provide solution-focused case management for DSNP Care at Home members in North Carolina. Telemedicine delivery and collaboration with the ICT are core parts of the role.

Reporting to the Social Work Manager, you will develop person-centered care plans, coordinate referrals, protect patient confidentiality, and participate in ongoing professional development.

Qualifications

• Master's Degree (MSW) in Social Work.
• Current unrestricted LCSW or LICSW license in the state of North Carolina.
• 2+ years of social work experience (internship not included).

Responsibilities

• Conduct comprehensive psychosocial assessments of patients and families.
• Utilize evidence-based screening tools for behavioral health conditions.
• Develop person-centered care plan goals with patients, families or caregivers.
• Facilitate serious illness and advance care planning discussions.
• Provide brief therapeutic interventions and solution-focused case management.
• Promote patient and family self-management strategies.
• Provide case management support and referrals for additional services.
• Facilitate communication among patients, families, and the interdisciplinary care team.
• Coordinate joint visits to support prognosis awareness and health literacy.
• Maintain patient confidentiality and pursue ongoing professional development.
