Optum's Lifeline Home Health Care division is seeking a Social Worker to provide medical social services under physician guidance. You will help patients and families understand social and emotional factors related to health and support coping strategies.

Responsibilities include assessing psychosocial needs, contributing to an integrated Plan of Care, and guiding the care team to connect patients with community resources as needed.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License and access to dependable transportation
