All Heart Home Care Agency seeks a per diem LMSW or LCSW to support NYC HRA DTS home care with psychosocial assessment and care coordination. This remote role requires NY licensure, CSWE-accredited MSW, and ability to work with complex cases from anywhere in the U.S.

Responsibilities include client assessment, staff training, referrals, documentation, and collaboration with interdisciplinary teams, while upholding HIPAA and ethics. Bilingual candidates are encouraged.

Qualifikationen

• Active, unrestricted New York State license and registration as an LMSW or LCSW required.
• MSW from CSWE-accredited program.
• Experience with home care, Medicaid-funded personal care, aging or disabled populations preferred.
• Comfort conducting assessments via telephone and video with confidentiality.
• Reliable, private workspace with HIPAA-compliant device for client communication.
• If outside NY: attest telehealth eligibility; licensure in home state may be required.
• Bilingual/multilingual candidates encouraged.
• Able to pass criminal background check and accommodate interpreters.

Aufgaben

• Provide psychosocial assessment and ongoing support for DTS clients with complex needs.
• Offer training and intensive support to home attendants/housekeepers on complex cases.
• Coordinate referrals to case management entities to connect needed resources.
• Help reduce turnover and case attrition through ongoing engagement.
• Identify risk factors and safety concerns; escalate per protocols.
• Maintain accurate case documentation per agency and NYS standards.
• Collaborate with nursing staff and administration as part of interdisciplinary care.
• Support QA/IP program with case reviews and service-delivery quality efforts.
• Maintain client confidentiality and adhere to HIPAA in all interactions.
