Job summary

All Heart Home Care Agency is seeking a per diem LMSW or LCSW to support the NYC HRA home care program, focusing on Difficult to Serve (DTS) cases. This is a remote position. Applicants must hold an active NYS LMSW or LCSW license and registration; you do not need to reside in NY but must meet

Qualifications

• Active, unrestricted New York State LMSW or LCSW license and registration.
• MSW degree from CSWE-accredited program.
• Experience with home care, Medicaid-funded personal care services, aging/frail elderly, and/or physically disabled populations preferred.
• Comfort conducting assessments and support sessions via telephone and video, with confidentiality and privacy.
• HIPAA-compliant device/setup and private workspace with secure internet.
• If residing outside NYS: attest to telehealth services compliance; NY licensure alone is not sufficient.
• Bilingual/multilingual candidates encouraged (Spanish, Russian, Haitian Creole).
• Ability to pass a criminal background check and provide eligibility documentation.

Responsibilities

• Provide psychosocial assessment and ongoing support for DTS clients with psychosocial, environmental, and family complexity.
• Offer training and intensive support to home attendants and housekeepers on complex cases.
• Coordinate referrals to case management entities (managed care plans, CASA offices, community services).
• Support efforts to reduce turnover and attrition on complex cases through engagement.
• Identify and address risk factors, safety concerns, changes in client condition, and potential abuse/neglect, escalating per protocols.
• Maintain accurate case documentation per agency, HRA, and NYS DOH standards.
• Collaborate with nursing and administrative teams as part of interdisciplinary care.
• Contribute to Quality Assessment and Improvement activities and case reviews.
• Maintain client confidentiality and comply with HIPAA and NASW Code of Ethics, including telepractice encounters.
