Job summary

Reliance Community Care Partners in Grand Rapids, MI is seeking a Social Worker or health services professional to serve as a Social Work Transition Navigator. The role offers flexibility, a home-office setup, and travel to participant locations with support from a team of navigators.

You will assess individuals, formulate care plans, coordinate with supports, help find housing, conduct safety evaluations, and advocate for services and benefits to enable safe community transitions.

Qualifications

• BSW or MSW licensure in Michigan (master’s preferred).
• Bachelor’s in health or human services with at least 3 years of experience.
• Community Health Worker certification with at least 3 years of experience.

Responsibilities

• Perform in-person assessments to identify desires, resources, strengths and barriers to transitioning into the community.
• Formulate an individualized plan of care to facilitate the transition.
• Coordinate with other supports and professionals to develop, initiate, facilitate and monitor the plan of care.
• Find housing options and coordinate with apartments, assisted living facilities, etc.
• Conduct or coordinate home safety evaluations and arrange transportation as needed.
