Job summary

Alliance Health in Morrisville, NC seeks a TCL Transition Coordinator to support members exiting institutional care into community living. You will coordinate services, housing and resources, traveling as needed and reporting onsite for meetings.

The role emphasizes collaboration with providers, teams and stakeholders to secure housing and health supports. The position requires a Bachelor’s in a human services field or RN with 3+ years’ experience in SMI/SED, with a preferred Master’s in Human

Qualifications

• Bachelor's degree in human services field or RN plus three years direct experience with SED/SMI.
• Preferred: Master's in Human Services and 1 year post-degree experience.
• Knowledge of community resources to address SDOH and housing is essential.

Responsibilities

• Assist treatment team with transitions from institutional care to community-based care.
• Review crisis and care plans to reflect member needs.
• Coordinate transition meetings and identify housing options.
• Monitor documentation to state requirements and manage caseload.
• Travel to Alliance offices and meet with members, providers, and stakeholders.
• Ensure compliance with DOJ settlements and DHHS transition guidelines.
