Placements24 is seeking a highly organized Social Services Coordinator to support individuals and families in need. This remote role focuses on assessing needs, developing tailored service plans, and coordinating access to a wide range of community resources.

You will advocate for clients, maintain confidential records, and collaborate with internal teams and external agencies to deliver comprehensive assistance that improves well-being in Randburg and the Gauteng region.

Qualifications

• Bachelor's degree in Social Work, Social Sciences, or related field.
• Proven experience in case management, social work, or community services coordination.
• Strong understanding of social welfare systems and resources in Randburg.
• Excellent assessment, planning, and problem-solving skills.
• Proficiency with digital communication tools and electronic records.
• Strong empathy, patience and ability to build trust with diverse client populations.

Responsibilities

• Conduct thorough needs assessments for clients seeking social services.
• Develop, implement, and monitor individualized service plans for each client.
• Coordinate the provision of services by liaising with internal teams and external agencies.
• Provide information, guidance, and support to clients regarding resources and benefits.
• Maintain accurate and confidential client records, documenting all interactions and progress.
• Advocate for clients' needs and ensure equitable access to services and support.
