Job summary

Placements24 seeks a dedicated Social Support Coordinator to provide remote assistance to individuals and families in need, connecting clients with social services and guiding them through applications. The role is contract, remote, and aimed at delivering compassionate, client-centered support via phone, email, and video calls.

You will conduct needs assessments, provide referrals to housing, food, and healthcare, support clients through application processes, and maintain confidential records

Qualifications

• Diploma or Certificate in Social Work or related field required.
• Experience in social support, case management, or counseling preferred.
• Knowledge of social welfare services in the local context.

Responsibilities

• Conduct remote needs assessments to understand client circumstances.
