Job summary
Allied Services in Pennsylvania seeks a Service Coordinator to respond to inquiries about home and community services for individuals with disabilities and their families. You will assist waiver recipients in accessing needed supports and coordinate services with the Waiver Coordination division.
Qualified candidates may hold an MSW/MSN or a BA in human services with related experience in assessments, nursing, or case management.
Qualifications
- Master's degree in Social Work (MSW) or Nursing (MSN) or related human services field, OR
- Bachelor's degree in human services plus relevant experience in psycho-social assessments, nursing, attendant care, or case management
Responsibilities
- Respond to inquiries about home and community services for individuals with disabilities and families.
- Assist waiver recipients in gaining access to needed supports and services.
- Coordinate and facilitate waiver services through Allied Services Waiver Coordination division.
- Ensure eligibility determinations and develop service plans with participants.
- Monitor health and safety of participants through visits and contacts.
- Provide participants with copies of service plans and notices of eligibility or non-eligibility.
