Service Access & Management, Inc. is seeking a Service Coordinator for the Community Wellness Service Line in Allegheny County, PA.

You will provide ongoing service coordination to Medicaid-eligible participants under the Community Health Choices waiver, ensuring health, safety, and wellness through coordinated plans and community linkages. The role requires a Bachelor’s in Social Work/ Psychology or RN, 3+ years in social/healthcare settings, and strong communication.

Qualifications

• Three or more years of experience in a social service or healthcare-related setting.
• Bachelor's degree in Social Work, Psychology, or related field with practicum experience.
• RN credential.
• Ability to work independently and manage a dynamic caseload.
• Strong interpersonal, organizational, and communication skills.
• Valid driver’s license and reliable transportation.

Responsabilités

• Engage in ongoing assessment of participants and complete comprehensive assessments.
• Refer participants to needed and/or approved services.
• Develop and maintain service plans outlining participant strengths and needs.
• Provide ongoing monitoring to ensure adherence to plans and safety of participants.
• Communicate priorities and information to participants and team members.
• Monitor Medicaid eligibility and coordinated supports for assigned participants.
• Foster positive working relationships with participants and team members to ensure ongoing support.

About The Service Coordinator Role

As a Service Coordinator within our Long-term Support Services team, you'll play a vital role in supporting participants who are Medicaid eligible under Community Health Choices waiver through providing ongoing service coordination services to eligible participants including ongoing monitoring to assure individual health, safety, and wellness; linkage to community and available resources; communicating information to participants; and developing and maintaining service plans outlining participant strengths, needs, and services . This position is ideal for someone who is compassionate, detail-oriented, and thrives in community-based environments where collaboration and relationships matter. and thrives in fast-paced environments where collaboration matters.

Starting Rate: $22.00/hour, per the current union agreement

Service Coordinator Core Responsibilities

• Engage in ongoing assessment of participants include completing comprehensive assessments.
• Ensure that participants are referred to needed and/or approved services.
• Develop and maintain plans that outline participant strengths as needs.
• Provide ongoing monitoring activities to ensure adherence to plans, and satisfaction with services, and determine the ongoing safety and well-being of participants served.
• Communicate priorities and information to participants and their identified team members.
• Service coordinators monitor and ensure that assigned participants maintain Medicaid eligibility.
• Service coordinators are responsible for developing positive working relationships with participants and identified team members to ensure that needed ongoing support is provided.

What You Bring

• Have at least three (3) or more years of experience in a social service or a healthcare-related setting
• have a bachelor's degree in Social Work, Psychology, or other related fields with practicum experience
• Register Nurse (RN)
• Ability to work independently and manage a dynamic caseload
• Strong interpersonal, organizational, and communication skills
• A valid driver's license and reliable transportation

Location & Schedule

This position is based in Allegheny County, PA. Work is primarily community-based with some remote flexibility.

Office hours are Monday through Friday, 8:30 AM to 4:30 PM.

Serivce Coordinators have on-call responsibilities.
