Job summary

Lucet is hiring a Senior Care Access Coordinator to ensure timely access to behavioral health services through advanced care coordination and member outreach. This non-clinical role involves cross-functional collaboration to connect members with providers and resources while removing barriers to care.

You will work in a remote, US-based position with a focus on delivering exceptional member experience and upholding HIPAA standards.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or related healthcare field.
• 2+ years in healthcare, managed care, behavioral health and/or customer service; OR high school diploma +4+ years in similar settings.
• Strong computer proficiency and ability to work across multiple systems.
• Ability to talk and type simultaneously during live member interactions.
• Working knowledge of behavioral health or medical terminology.
• Strong problem solving, organizational skills and the ability to prioritize multiple tasks.
• Ability to pass comprehensive background checks.

Responsibilities

• Conduct proactive outreach and post-hospitalization follow-up to connect members with behavioral health services and coordinate appointments.
• Build trusted member relationships to assess needs and identify providers and resources, addressing barriers to care.
• Partner with clinicians and internal teams to support high-risk member follow-up and continuity of care.
• Educate members, providers, and facilities on available services and care management processes.
• Maintain accurate, timely documentation in HIPAA-compliant systems and meet performance goals.
