Job summary

Corewell Health Southfield Center is seeking a part-time professional to coordinate multi-discipline services within and outside the health system. You will exercise independent judgment in assessment, diagnosis initiation of selected protocols, and provide educational support for patients, families, and staff.

• 2 years of relevant experience required.

Responsibilities

• Coordinate services provided by multiple disciplines within and outside the health system.
• Provide educational support for patients/significant others, health team members, and the community; coordinate evaluation practices to measure outcomes of programs.
