Corewell Health is seeking a part-time day-shift role focused on coordinating services across multiple disciplines within and outside the health system. You will assess, initiate selected protocols, and provide education to patients, families, and the health team while measuring program outcomes.
The role requires a Bachelor’s degree (Master’s preferred) and at least 2 years of relevant experience, with licensure applicable to Michigan health professions.

Scope of Work
Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.
