CVS Health is seeking a Transition of Care Case Manager (TOC CM) in Illinois to lead seamless care transitions for high-risk, medically complex members enrolled in Medicaid waivers and SNP programs.

You will perform comprehensive discharge planning, coordinate with the Interdisciplinary Care Team, and support members and families with education and resources to reduce readmissions and improve health outcomes, in partnership with providers and community organizations.

Qualifikationen

• Active and unrestricted RN license in Illinois or ability to obtain.
• 3+ years of clinical practice experience.
• 1+ year in care coordination or high-risk populations.
• Proficient in MS Office; Word, Excel, Outlook, OneNote, Teams.
• Ability to work from home with independent, remote collaboration.
• Private dedicated space to work effectively.

Aufgaben

• Complete market-specific post-discharge assessments, including SDoH.
• Provide discharge planning and care transitions between settings.
• Ensure medication availability and patient understanding of meds.
• Coordinate ICT meetings to develop ICPs and collaboration with care teams.
• Identify barriers to optimal member health and mitigate risks.
• Support communication with interdisciplinary teams and providers.
• Ensure compliance with state and federal regulations.
• Lead and participate in care team planning and care coordination.
