Community Physicians in Winfield, IL is seeking a Hospital Case Manager (RN) to orchestrate safe, timely transitions from hospital to post-acute care for medically complex older adults. You will lead discharge planning, collaborate with physicians and therapists, and advocate for resources to avoid readmission.

The role requires BSN, an active Illinois RN license, and 3–5 years in discharge planning or case management.

Qualifications

• BSN required.
• Active, unrestricted RN license in Illinois.
• 3–5 years of hospital discharge planning or case management experience.
• Strong knowledge of Medicare/Medicaid guidelines and local resources.

Responsibilities

• Lead discharge planning with physicians, therapists, and nursing staff for geriatric patients.
• Conduct assessments to identify barriers to safe discharge.
• Coordinate post-acute services including SNF, Home Health, Rehabilitation, and Hospice.
• Document in EMR to comply with regulations.
• Counsel families on elder care and chronic disease management.
