CVS Health is seeking a Transition of Care Registered Nurse to support high-risk, medically complex members through safe transitions across care settings.

This remote role in Illinois offers a full-time schedule with pay from $66,575 to $142,576 per year. Requires an Illinois RN license and care coordination experience, with responsibilities including discharge planning, post-discharge assessments, and patient education for caregivers.

Qualifications

• Associate’s Degree in Nursing is required.
• 3+ years of clinical practice experience.
• 1+ year in care coordination or with high-risk populations.
• Active and unrestricted RN license in Illinois or willingness to obtain.
• Certified Case Manager is preferred.

Responsibilities

• Perform market-specific post-discharge assessments to identify member needs.
• Provide discharge planning and coordinate transitions of care.
• Ensure members understand and have access to prescribed medications.
