Job summary

Sagility is seeking experienced Registered Nurses to join our team as Senior Care Specialists in a work-from-home capacity. This seasonal opportunity has a tentative end date in June 2027 and may include bonuses or incentives determined by leadership after joining Sagility.

You will assess members’ needs by phone, develop individualized care plans, provide education and support, and coordinate services to improve health outcomes using a Whole Person Care approach.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years clinical nursing experience, 3+ preferred.

Responsibilities

• Conduct telephonic health assessments for members.
• Develop individualized care plans based on health risks.
• Provide education and support for chronic condition management.
• Coordinate services and transitions of care.
• Document interactions and outcomes in applicable systems.
