Job summary

Sagility is seeking experienced and compassionate Registered Nurses (RNs) to join our team as Senior Care Specialists in a work-from-home capacity, supporting members with chronic conditions.

This seasonal position offers flexible telework, clinical autonomy, and opportunities for bonuses based on performance. You will assess needs, develop care plans, educate members, and collaborate with a multidisciplinary team to improve health outcomes.

Qualifications

• Active, unrestricted RN license in state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency including Word, Excel, and Outlook.
• Ability to efficiently navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education related to chronic condition management.
• Use nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and discharge planning, including DME and home health services.
• Help members navigate healthcare benefits, services, and care options.
• Assess factors that affect health and well-being.
• Collaborate with multidisciplinary teams to support member care.
• Promote self-management with education and resources.
• Coordinate care activities among members and care team.
• Document interactions and outcomes in applicable systems.
