Job summary

ResPro Health is hiring a Remote Registered Nurse to support patients in chronic pulmonary care programs from home. You will educate patients, develop personalized care plans, and coordinate with respiratory therapists and providers to optimize outcomes.

The role requires an unrestricted RN license in GA, FL, TX or SC, at least 3 years in adult health, EMR experience, and strong communication skills. Remote work with collaborative team support is offered.

Qualifications

• Active and Unrestricted Georgia, Florida, Texas or SC RN License (Compact License preferred).
• At least three years of experience in adult health preferred.
• Background in adult chronic health conditions (Pulmonology, Cardiology, Care Management) preferred.
• Variety of Electronic Medical Record (EMR) experience.
• Proficient knowledge, skill, and interest in basic computer skills.

Responsibilities

• Welcome patients into continuous care program(s) and review benefits and services included.
• Coordinate with the patient's Respiratory Therapist and pulmonary providers to ensure a collaborative approach to care.
• Educate patients on the frequency and use of their in-home monitoring devices, if applicable.
• Create a personalized, comprehensive care plan with the patient via phone or video visits.
• Identify and address barriers to patient success.
• Provide education and coaching on chronic pulmonary conditions.
• Review readings and update the plan of care monthly with the patient.
• Serve as the first contact for non-emergent needs (medication refills, scheduling, reminders).
• Assist patients in navigating healthcare (appointments, coordinating with specialists, screenings).
• Review in-home device readings in real time during business hours.
• Follow protocols for documenting device trends and symptoms.
• Escalate concerning readings to the provider per protocol.
• Establish rapport to motivate positive changes in health.
