ResPro Health, LLC is seeking a Registered Nurse to support patients remotely in chronic respiratory care programs. You will connect with patients daily from home and coordinate care with Respiratory Therapists and pulmonary providers.

Responsibilities include educating patients on home monitoring devices, creating personalized care plans, and serving as the first point of contact for non-emergent needs. The role emphasizes patient engagement, follow-ups, and collaboration with a

• Active and unrestricted RN license in GA, FL, TX or SC.
• At least three years of experience in adult health preferred.
• Background in adult chronic health conditions (Pulmonology, Cardiology, Care Management) preferred.
• Variety of EMR experience.
• Proficient in basic computer skills.
• Strong problem solving and multitasking abilities.
• Excellent written and verbal communication and teamwork.

Responsabilidades

• Welcome patients into continuous care programs and review benefits.
• Coordinate with Respiratory Therapist and pulmonary providers for collaborative care.
• Educate patients on home monitoring devices and their usage.
• Create personalized care plans via phone or video visits.
• Identify and address barriers to patient success.
• Provide education and coaching on chronic pulmonary conditions.
• Review readings and update care plans monthly.
• Serve as first contact for non-emergent needs (meds, scheduling, reminders).
• Assist with navigating healthcare and coordinating with specialists.
• Review in-home device readings in real time during business hours.
• Escalate concerning readings to the provider per protocol.
• Establish rapport to motivate positive health changes.
