Job summary

ResPro Health, LLC is seeking a skilled Registered Nurse to join our remote patient care team. In this role, you will support patients in remote monitoring and chronic care management, coordinating with Respiratory Therapists and providers, and educating patients on device use and care plans.

You will review readings, maintain ongoing patient communication, assist with scheduling and referrals, and ensure care plans are updated monthly. A strong RN license and EMR experience are required.

Qualifications

• Active and Unrestricted RN license in GA, FL, TX or SC.
• At least 3 years of experience in adult health preferred.
• Background in adult chronic health conditions (Pulmonology, Cardiology, Care Management) preferred.
• Variety of Electronic Medical Record (EMR) experience.
• Proficient knowledge, skill, and interest in basic computer skills.
• Proficient in problem solving and ability to multi-task.
• Excellent communication skills (oral and written).
• Excellent teamwork skills.
• Clean background check and drug screening.
• Comfortable working remotely but collaboratively.

Responsibilities

• Welcome patients into continuous care program(s) and review benefits and services included.
• Coordinate with the patient's Respiratory Therapist and pulmonary providers to ensure a collaborative approach to care.
• Educate patients on the frequency and use of their assigned in-home monitoring devices, if applicable.
• Create a personalized, comprehensive care plan with the patient via phone or video visits.
• Identify and address any barriers to patient success.
• Provide specific education and coaching on patients' chronic pulmonary conditions.
• Connect with the patient frequently to review readings and update their plan of care monthly.
• Serve as the patient's first contact for all non-emergent needs (e.g., medication refills, scheduling).
• Assist patients in navigation of their healthcare (scheduling appointments, coordinating with specialists).
• Review and evaluate in-home device readings in real time during normal business hours.
• Follow established protocols for documenting device trends and any symptoms in medical records.
• Escalate concerning readings and/or symptoms to the provider per protocol.
• Establish a rapport that builds trust and motivates positive changes in health.
