Job summary

ResPro Health, LLC is seeking a Registered Nurse to support patients in home-based remote monitoring and chronic care management programs. You will coordinate with Respiratory Therapists and pulmonary providers, educate patients on device use, and develop personalized care plans via phone or video visits.

This remote role requires an active RN license in Florida (compact license preferred) and at least three years of experience in adult health.

Qualifications

• Active and Unrestricted Florida RN license (Compact preferred).
• At least three years of experience in adult health.
• Background in pulmonology, cardiology, or care management preferred.
• Proficient in EMR systems and basic computer skills.
• Excellent communication and teamwork abilities.
• Ability to work remotely with self-discipline and reliability.

Responsibilities

• Welcome patients into continuous care programs and review benefits.
• Coordinate with Respiratory Therapists and providers for collaborative care.
• Educate patients on in-home monitoring devices and usage.
• Create personalized care plans via phone or video visits.
• Identify and address barriers to patient success.
• Provide education on chronic pulmonary conditions.
• Review readings with patients and update care plans monthly.
• Be the first contact for non-emergent needs (refills, scheduling, reminders).
• Assist patients with navigating healthcare and coordinating with specialists.
• Review in-home device readings in real time during business hours.
• Document device trends and symptoms in the medical record.
• Escalate concerning readings to the provider per protocol.
• Establish rapport to motivate positive health changes.
