Job summary

Carolina Family Care, Inc. (CFC) is seeking a Nurse Care Coordinator for its PCMH program. The role involves coordinating care for an assigned patient population, working closely with physicians, staff and care teams to ensure timely, appropriate care.

The position is primarily remote with travel to clinics as needed, requiring residency in South Carolina. The ideal candidate has a BSN, 3 years of nursing experience, an SC (or compact) RN license, and BLS certification.

Qualifications

• Bachelor’s degree in nursing required; three years of nursing experience.
• RN license by the South Carolina Board of Nursing or a compact state.
• BLS certification (AHA or Red Cross) required.
• Some positions may require ANA generalist certification.

Responsibilities

• Identify patients qualifying for care coordination and plan follow-up actions.
• Outreach to patients for CCM, quality measures, and referrals.
• Utilize Epic registries to identify needs and document attempts and completion.
• Coordinate scheduling, referrals, tests, and follow-ups as appropriate.
• Maintain data accuracy and summarize information for quality measures.
• Communicate effectively with patients, care team, and providers.
