Your Health Organization in South Carolina seeks a Registered Nurse to join our urgent care team, delivering coordinated care across homes, assisted living facilities, clinics, and virtual visits. You will collaborate with a provider and specialist department to support patients in your assigned service area.

Essential duties include care coordination, CCM plan management, telehealth facilitation, and collaboration with hospice teams to ensure comprehensive patient care.

Formación

• Must be a licensed nurse with license in good standing.
• Minimum of three (3) years clinical experience preferred.
• Ability to supervise and direct professional and administrative personnel.
• Ability to read and communicate effectively.
• Ability to manage and demonstrate effective leadership skills.
• Strong interpersonal and communication skills under all conditions.

Responsabilidades

• Admission
• Coordination of care and communication, collaboration with healthcare team
• CCM plan of care review and revision
• Facilitate telehealth visits with nurse practitioner for care coordination
• Coordinate with hospice interdisciplinary team and others in patient care plan
• Participate in hospice IDG meetings
• Coordinate with Assisted Living and Skilled Nursing Facilities teams
• Collaborate with physicians, therapists, home health, hospice to support patient plan
• Assess and chart patient condition observations
• Ensure follow-up visits are scheduled
• Participate in quality improvement projects (e.g., root cause analysis)
• Maintain updated provider/specialist lists in EMR
• Communicate with patients, families, and caregivers
• Utilize company software and update information as required
• Participate in coaching calls
• Perform other duties as required by SC House Calls
