Job summary

Allegheny Health Network seeks an RN Nurse Navigator to coordinate care for adult patients within specialized disease states. You will collaborate with a multidisciplinary team to connect patients with clinical and supportive services and guide them through treatment options.

The role emphasizes outreach, education, care coordination, and ensuring smooth transitions between care settings. A PA RN licensure (or eNLC) and 1–3 years of nursing experience are required.

Qualifications

• Current State of PA RN licensure or multi-state licensure through eNLC.
• 1-3 year nursing experience in the specialty required.

Responsibilities

• Outreach to high-risk populations and disease-specific sites to build and sustain relationships within the network.
• Educate patients on diagnosis, treatment options, and care resources.
• Coordinate and ensure safe handoff between facilities and care settings.
• Assist Registry staff with data collection and care-delivery improvements.
• Coordinate appointments with the multidisciplinary team and support medication adherence.
