Allegheny Health Network is seeking an RN Nurse Navigator to coordinate care for adult patients across AHN Wexford and Grove City Hospital locations. You will serve as the primary contact for patients, families, and referring physicians, arranging appointments and coordinating with the multidisciplinary team to ensure seamless transitions of care.

You will educate patients on diagnoses and treatment options, identify community needs, and facilitate referrals and resources.

Qualificações

• Valid PA RN license or multi-state eNLC licensure.
• 1–3 years of nursing experience in a relevant specialty.

Responsabilidades

• Serves as the primary contact for referring physicians, patients, and families.
• Coordinates patient flow, including appointment coordination and access to services.
• Collaborates with the multidisciplinary team to ensure comprehensive care.
• Educates patients on diagnosis, treatment options, and available resources.
• Identifies community needs and facilitates screening, referrals, and care coordination.
• Trends data and outcomes for the navigation program and reports to the team.
